Food Establishment Renewal
If you are a Food Establishment looking to renew your Food Establishment Permit please fill out an updated Application then Mail or Drop Off Application with your Permit fee to our office. Please include a copy of your Certified Food Handlers Certificate and copy of your Photo ID.
Food Establishment Permits are $100 and run through the end of the calendar year. There will be a $50 late fee Assessed for any permit renewals obtained after January 5th.
New Food Establishment Applicants
STEP 1
Print and fill out the Plan Review Instructions, Plan Review Questionnaire, and Application for Plan Review documents. Fill out and bring into the Ripley County Health Department for our Chief Food Inspector to review. You will be notified when your plan is approved.
STEP 2
After your Plans have been approved, you will then need to print and fill out the Food Establishment Permit Application. Return this application along with a copy of your Photo ID and a copy of your Certified Food Handlers Certificate to the Ripley County Health Department.
